CMSRN certification: the med-surg credential, explained

Facts checked against msncb.orgLast checked Nov 4, 2026

CMSRN stands for Certified Medical-Surgical Registered Nurse. It is the med-surg specialty certification awarded by the Medical-Surgical Nursing Certification Board (MSNCB), and, unlike the CEN, it has a practice-hours gate (RN + 2,000 med-surg hours). Pass the exam and the credential runs 5 years before renewal.

Who qualifies: the hours rule

Two things are checked: an RN license in good standing, and med-surg practice hours inside the past three years. The setting rule trips applicants up: hours in pediatrics, psych, OB/GYN, the OR or the ED do not count, whatever the job title. A float nurse who splits shifts between a med-surg floor and the ED counts only the floor shifts, so total them from your schedule before you apply.

Two years of practice is suggested but not required, and no BSN is needed. If you are licensed outside the US or Canada, confirm the hour count with MSNCB before applying: its handbook and website give different figures. A supervisor confirms the work history, and MSNCB audits a share of applications at random (registration steps).

MSNCB, AMSN and the letters after your name

You apply to MSNCB, the certification board. AMSN is the membership association tied to it, and membership only lowers the price. The letters CMSRN after your name mean you passed MSNCB's exam, not ANCC's med-surg exam.

CMSRN vs MEDSURG-BC

Search "med surg certification" and two credentials come up: this one and MEDSURG-BC, which ANCC awards (ANCC's page).

  • Separate exams. Each board writes its own test and runs its own application, fees and renewal. Passing one does not earn the other.
  • Check before you choose. If a job posting, clinical ladder or Magnet document names one credential, take that one. Where either is accepted, compare the current requirements on both boards' pages.

MEDSURG-BC is not on First Attempt yet.

Renewal every 5 years

Requirements: 1,000 med-surg practice hours in 5 years on either route, plus 90 contact hours (at least 68 med-surg, at most 22 professional development) or a passing exam score.

Full-time RN students in a BSN, master's or doctoral program can document enrollment instead of the hours. Renewal opens in year 4. 10% of renewals are audited. A 1-year grace period carries a $90 late fee; after it only the exam restores the credential.

Fee: $325 by contact hours for non-members ($225 AMSN members); by exam $394 / $267.

The practice-hours part is easy to forget. A nurse who moves off the floor into education or management mid-cycle should check early whether the new role still adds med-surg hours.

Which discharge recommendation is an appropriate nonpharmacologic adjunct for chronic musculoskeletal pain?

Use Check on the answer you favor. The verdict shows up with a short note on each choice.

  1. Check
Jumping into high-impact aerobic work with no pacing tends to provoke a flare, and clients then abandon activity altogether.
  2. Check
Graded activity rebuilds tolerance without flaring the pain, and heat or cold is a low-risk measure the client can keep using at home.
  3. Check
Prolonged bed rest deconditions muscles and joints, and routine opioid escalation is a drug plan, not a nonpharmacologic adjunct.
  4. Check
Acupuncture booked on a standing schedule with no assessment skips individualization; a discharge recommendation has to fit this client's condition and resources.
2/4 · Teamwork · 21%

A nurse manager wants to strengthen ongoing professional growth among unit staff. Which strategy is most effective?

  1. Check
Yearly mandatory modules keep minimum competency; restricting education to them caps growth at the required baseline.
  2. Check
Posting individual metrics publicly shames staff and erodes trust; performance data supports growth when it is shared privately with coaching.
  3. Check
Peer review gives each nurse specific feedback from colleagues on their own practice, which builds accountability and keeps learning continuous instead of annual.
  4. Check
Unpaid mandatory overtime for training breeds fatigue and resentment and conflicts with fair labor practice, so it undermines engagement.
3/4 · Interprofessional · 17%

Disciplines disagree over who provides post-discharge wound teaching. Which step best ensures a safe discharge?

  1. Check
A short huddle settles who owns the wound teaching before the client leaves, so the gap is closed and everyone knows the plan.
  2. Check
Charting the disagreement records the problem but assigns no one to teach, so the client goes home without wound-care instructions.
  3. Check
Going to administration before the team has talked skips the quickest fix and turns a simple role question into a conflict.
  4. Check
If each discipline decides on its own, the teaching is duplicated or missed entirely, which is the original problem.
4/4 · Holistic care · 15%

A nurse is starting an advance care planning conversation with a client who has advanced COPD. Which statement by the nurse is the best opening?

  1. Check
Asking about 'everything' at cardiac arrest forces a yes-or-no choice on a frightening scenario before the client's goals are known.
  2. Check
Opening with the DNR form jumps to paperwork and signals that a decision is expected, which tends to shut the conversation down.
  3. Check
Handing the topic to the doctor closes the conversation; nurses can and should begin advance care planning discussions.
  4. Check
Asking what matters most is an open question that invites the client to name values and goals first, which gives later choices about specific treatments a foundation.

CMSRN: quick answers

What does CMSRN mean?

Certified Medical-Surgical Registered Nurse: an RN who has passed the med-surg certification exam described here and keeps the credential current.

Is the CMSRN the same as MEDSURG-BC?

No. They are separate credentials from separate boards; see the comparison above.

Can the CMSRN lapse if I leave med-surg?

It can. Renewal asks for med-surg practice hours during the cycle, so a move off the floor needs planning; see renewal.
